Terms and conditions for One Class Per Week Membership

ONE CLASS PER WEEK MONTHLY MEMBERSHIP AGREEMENT

This contract made between the client and FIT4MOM Hudson, Twinsburg, and Streetsboro is a month to month agreement (after initial 3 month commitment) except as provided in the initial 5 day cancellation policy below, may be terminated by either party with the proper 1 week notice prior to billing date. This membership allows you to attend one of our fitness class per week.

AUTHORIZATION FOR PAYMENT

I authorize FIT4MOM Hudson, Twinsburg, and Streetsboro to initiate a charge to the credit card number entered into Front Desk for my dues. This authorization is to remain effective until I have provided written notice 1 week prior to next billing date to cancel my membership through the FIT4MOM franchise owner. Regular membership fees apply until the last 30 days of membership expire. Any plan that is purchased with a waived membership fee requires a 3 month commitment.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Hudson, Twinsburg & Streetsboro to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a return fee of $25. Members are responsible for updating their card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

CANCELLATION POLICY

To cancel this membership, the member must contact the FIT4MOM franchise owner in writing 1 week prior to their next payment due date to request a cancellation. Regular membership fees will be collected for the last 30 days of membership. No refunds will be given for dues already paid. This membership is not transferable to another person or anywhere outside the Hudson, Twinsburg, and Streetsboro franchise. If cancelled before the 3 month commitment is met, the member will be charged the remaining amount to meet all required monthly payments.

HOLD POLICY
Memberships may be held a maximum of 3 months per hold period, not to exceed one hold per year. Holds can start and end on any day of the month. When a hold period is ended, the monthly auto payments will resume on the return date. There will be a $25 processing fee charged to your credit card on file for the hold period, and a member must give 1 week written notice if canceling the membership after a hold. By holding a membership, a member is waiving her participation in any fitness classes but may continue to attend private FIT4MOM social activities, such as playdates, private Mom's Night Outs, and family activities.

INITIAL 5-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the fifth calendar day after you have signed this agreement. To cancel this agreement, written notice must be received by FIT4MOM Hudson, Twinsburg, and Streetsboro within the initial five days. You are responsible for paying for the classes already attended.

AGREEMENT

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
